RESIDUALS PAYMENTS FOR "MISSING BENEFICIARIES"
You may be entitled
to Residuals!
The DGA may be looking for you as one of a number of beneficiaries whose residuals checks have been returned to the Guild due to an incorrect address, or as the legal beneficiary of a deceased member for whose estate the Guild is holding residuals.
The DGA website will be listing the names of people whom the Residuals Department is holding checks for. Use this website to check to see if you or someone you know are among the "missing," and urge your fellow members to do the same. If your name — or that of a deceased member you knew — appears on the website, then you, or the estate or heir may be entitled to receive residuals payments. To verify your entitlement to residuals, you must complete the questionnaire available on the website and return it to the DGA with the necessary substantiating documents.
DO NOT TELEPHONE. Telephone requests for the questionnaire will not be honored.
The questionnaire contains filing instructions and information regarding the required substantiating documentation. The DGA appreciates your patience and will make every reasonable effort to provide a response within 30 days after our receipt of the completed questionnaire and all required substantiating documentation.
