Although the DGA sends members' residuals to the most current address on file at the Guild, W-2s (Wage and Tax Statement forms) are sent directly to the members by the entities that have written those checks. However, the studio or payroll house may not have the most current address for a member. If you are missing a W-2 form, or need to change your address so future W-2s will be delivered to the correct address, please consult the chart below for Change of Address fax lines and/or W-2 Hotlines for several studios and payroll houses. If the company issuing your W-2 is not on the list, please call their main number.
